It is almost time for students to get their sports physicals for the 2013-2014 school year.  For some activities such as, marching band, football, volleyball, and cross country, forms need to be turned before the last day of school.  Remember to bring the physical form to your appointment.  Have the patient medical and family history sections completed before your appointment.  Wear loose fitting shorts to your appointment.  To schedule a sports physical, call the office at (281)292-0999.
